Our group of experienced Nurse Care Managers work with both individuals and groups to help them improve their health along a continuum from birth to end of life.  We help people maintain and improve their current health status by emphasizing healthy lifestyle, educating them about better health and coaching them on wellness.  When disease processes are already involved, we assist people to achieve optimal wellness.  Sunrise-Sunset Care Managers are seeking to bring personalized wellness services to their clients.
